Primary Resident Application

Primary resident requirements:

Legally blind

Overall health and needs are appropriate for the level of care we provide. Someone in need of skilled nursing care would not be accepted, nor would someone who simply needs an affordable place to live.

Age 22 years or over

There is a two-step admission process.

First, the applicant (or representative) fills out an application and then speaks to one of our staff. If the applicant is still interested in proceeding Friedman Place will make a referral to someone working for the State of Illinois to conduct a brief assessment that mostly focuses on functioning, for example, how the applicant manages cooking, cleaning, laundry, travel, etc. This assessment occurs in the applicant’s own home and helps answer the questions “Does the applicant appear eligible for supportive living? Do they need the services that supportive living provides?” Infrequently, there are two assessments done by two different people working for the State of Illinois. It can take one to four weeks to schedule the assessment. There is no cost or obligation to applying.

The second step: The second step, which occurs after the assessment conducted by the State of Illinois, finds that the applicant appears eligible, typically involves 1-2 visits to Friedman Place and an interview by a social worker and a nurse. They ask a wide range of health and related questions to determine if the applicant is appropriate for Friedman Place. Additionally, medical and other records are requested for further review. If accepted, the individual is then placed on the Waiting List until an apartment is available. There is no cost or obligation to applying.

NOTE An informational visit to and tour of Friedman Place is available to anyone at any time, whether or not they have completed an application or begun the assessments.

Friedman Place is “all inclusive," and a single monthly payment covers everything, including nursing and personal care, social work services, activities and outings, housekeeping, maintenance services, a private apartment with full bathroom and kitchenette, utilities, internet, cable tv, three meals per day plus a snack, basic telephone services, and more. Most residents receive Medicaid insurance coverage which substantially subsidizes their costs.

Guide dogs are welcome and encouraged and Friedman Place will help pay for the costs of food and grooming if needed. The only pets that are permitted are fish and birds.
Yes, as long as your spouse (or partner, boyfriend, girlfriend, etc.) qualifies like any other applicant. Studio apartments are only for individuals; one bedroom apartments (when available) are for couples.
We go shopping weekly and typically (depending on the season) go on another 1-2 outing per week.
Yes, with a full bathroom and kitchenette.
Your one monthly payment includes the apartment, all utilities, meals, housekeeping, activities, phone service, cable tv, assistance from nurses, certified nursing assistants, social workers, etc.
